The stroke department has been operating since February 2012. The department provides round-the-clock urgent assistance to the population of the Kyzylorda region and the city. The office is located on the 2nd floor in Block F, with a capacity of 50 beds, 18 chambers, including 6 PIT chambers. The department is highly specialized. Patients with acute cerebrovascular accident (stroke) receive treatment in the neurostroke department.

Stroke is an acute violation of cerebral circulation, characterized by a sudden (within a few minutes, hours) appearance of focal and / or cerebral neurological symptoms, which persists for more than 24 hours or leads to the death of the patient in a shorter period of time due to cerebrovascular pathology. Strokes include cerebral infarction, cerebral hemorrhage, and subarachnoid hemorrhage [1], which have etiopathogenetic and clinical differences. Given the time of regression of neurological deficit, transient cerebrovascular accidents (neurological deficit regress within 24 hours, unlike the stroke itself) and minor stroke (neurological deficit regress within three weeks after the onset of the disease) are especially distinguished. Vascular diseases of the brain take the second place in the structure of mortality from diseases of the circulatory system after coronary heart disease [2] [3] Doctors of the department provide highly qualified specialized care in acute subacute periods of stroke, starting from the first minutes of the patient’s admission to hospital. The department’s work is carried out in close collaboration with the Department of Neurosurgery, the Department of Cerebrovascular Pathology, and the Department of Neuroanimation and Endovascular. Hospitalization of patients on the first day from the onset of the disease according to the indications and contraindications. According to the order of the Ministry of Health of the Republic of Kazakhstan No. 382 dated April 17, 2012, the joint venture team can be taken to the stroke department and Kyzylorda residents hospitalized for the thrombolysis procedure. If the results of the examination of the patient according to the protocol exclude the use of the thrombolysis procedure, such a patient remains in the Stroke Department for the treatment of acute brain pathology according to standard protocols. The department is the base of excellence for the neurologists of Kyzyorda. Since 2012, the neurostroke department has been recognized and remains the leader in the application of modern treatment methods (thrombolytic therapy) for patients with acute ischemic stroke and interventional endovascular interventions on cerebrovascular vessels. Upon admission to the department, the patient is examined by a neurologist on duty and examined according to the standard protocol developed by the Expert Council of the Ministry of Health of the Republic of Kazakhstan dated 04/17/2013:

1. brain CT is performed;

2. X-ray examination - radiography of the lungs;

3. Radiography of the cervical spine in 2 projections;

4. Clinical and biochemical examinations;

5. ECG;

6. Holter monitoring of blood pressure and ECG;

7. Duplex scanning of extracerebral vessels;

8. If necessary - MRI examination of the brain in tissue and vascular modes.

Patients with hemorrhagic stroke and precerebral vascular pathology are consulted by a neurosurgeon, after which, together with the head of the cerebrovascular neurosurgery center, the optimal tactics of further patient management is determined - either the continuation of conservative treatment or the need for surgical intervention. The department provides daily assistance to patients by a multidisciplinary team. Much attention is paid to round-the-clock patient monitoring. All patients, regardless of the state of admission, are in the intensive care wards for the first day, where round-the-clock monitoring of the main hemodynamic indicators and vital signs is carried out, round-the-clock supervision is carried out by staff. All actions of the duty personnel are provided for by the expert council of the Ministry of Health of the Republic of Kazakhstan, which regulates the list and scope of actions of the doctor on duty and mid-level personnel in a stroke department. This organization of the stroke unit allows you to quickly respond to changes in the patient's condition and provide the necessary assistance in time. Round-the-clock surveillance and early activation of patients helps prevent complications of the course of stroke - pulmonary embolism, myocardial infarction, congestion in the lungs - can reduce the degree of neurological deficit. When the condition stabilizes and the dynamics of the neurological status are positive, the patient is transferred to the neurorehabilitation department for further treatment and rehabilitation or is discharged at the place of residence under the supervision of the district / family doctor. Upon discharge from the department, the patient receives: 1. an extract from the medical history - comprehensive information about his state of health, 2. results of examinations and recommendations for the secondary prevention of stroke according to modern views on the problem of stroke, 3. instructions for caring for a patient with a severe neurological deficit at home, 4. recommendations for controlling blood pressure and a cholesterol-free diet, 5. leaflet “How to prevent a stroke”, 6. list of necessary medications for secondary prevention of acute usheny cerebral circulation, with a clear provision of the dose and duration of drug use. Considerable attention is paid to conducting educational work among the population, thematic broadcasts are conducted on regional television, articles are published in regional and city newspapers on assisting patients with stroke in the Kyzylorda region, and educational work is constantly being conducted with patients and their relatives on the topic “Life after a stroke” "And how to avoid a stroke. The plans of the neuro-stroke department will introduce throughout the year daily video information in the department on the prevention of cerebrovascular accident and the promotion of a healthy lifestyle. Doctors of the department are constantly working on their professional level, participate as speakers and listeners in regional, republican and international conferences on cerebrovascular pathology, have printed works on providing assistance to patients in the acute period of stroke in medical journals of the Republic of Kazakhstan. Thanks to close cooperation with the Department of Nervous Diseases of the National Medical University named after Asfendiyarov, RSCC Astana, the process of treating patients with stroke in the Stroke Department is dynamic and creative, which is constantly attracted to the latest techniques and recommendations.
